Pre-Cover Inspection Request
For all systems permitted after September 18, 2023, inspection requests can be made in one of the following ways:
- Scheduling online (Instructions for scheduling)
- Using ePermitting Apps to schedule – Android and iPhone
- IVR (Interactive Voice Response) – phone menu system
- SelecTXT – scheduling via text messaging
Pre-cover inspections of septic systems installed prior to September 18, 2023 may be requested by completing this form, or by calling our pre-cover inspection request line at (541) 967-3821.
*A completed and signed As-Built form must be submitted to our office in addition to any other paperwork specified in your permit. The Certificate of Satisfactory Completion cannot be issued until all required documents are received. Documents may be attached to this request form, left onsite or submitted via email to: precoverrequest@linncountyhealth.org
