Community Health Improvement Process

The Community Health Improvement Process allows the opportunity for our community to work together to improve health for all in Linn County. The process includes the development of a community health assessment (CHA) and a community health improvement plan (CHIP). As an accredited Public Health agency, Linn County Public Health, in partnership with the community, conducts a CHA and a CHIP  every 5 years. Community Health Assessment (CHA)

The CHA provides an opportunity for all to learn about the health of our community and understand the issues that influence our health. The CHA data helps to identify opportunities for improvement, and discuss what we, as a community, can do to improve our health. The 2022 Linn County Community Health Assessments can be found in the documents below. 

Community Health Improvement Plan (CHIP) 

A CHIP is a long-term, systematic effort to address public health problems based on the results of a community health assessment (CHA) and a community health improvement process. CHIPs help to provide support for developing new policies and determining health promotion strategies. It should define a shared vision for community health through collaboration with diverse partners and address the broad range of strengths, weaknesses, challenges, and opportunities that exist within the community to improve health. 

2024-2028 Linn County Community Health Improvement Plan 

The Partnership for Community Health includes Linn, Benton, and Lincoln county health departments, Confederated Tribes of Siletz Indians, Intercommunity Health Network Coordinated Care Organization (IHN-CCO), Samaritan Health Services, and United Way of Linn, Benton, and Lincoln Counties.

2024- 2028 Linn County Community Health Improvement Plan 

The priority areas of the 2024-2028 Linn County CHIP are: 

  • Access to Affordable Housing: The long-term vision of these goals is to ensure that all Linn-Benton-Lincoln residents have safe, affordable housing with a focus on priority populations who have been economically and socially marginalized.
  • Access to Quality Care: The long-term vision of these goals is to improve health equity and population-level health outcomes by ensuring that all Linn-Benton-Lincoln residents can access the right care at the right time and place.
  • Behavioral Health: The long-term vision of these goals is to ensure that all Linn-Benton-Lincoln residents have equitable access to behavioral health support and treatments.
  • Inclusion, Diversity, Anti-Racism, and Equity (IDARE): The long-term vision of these goals is to transform health and public health systems to increase inclusion, well-being, and community participation and engagement in the decision-making processes.
